CPC is seeking a Care Coordinator who will work alongside our multidisciplinary team which delivers high quality, evidence informed, and community-based behavioral healthcare services to our clients. The Care Coordinator will work directly with clients with chronic medical and behavioral health conditions in order to reduce Emergency Room visits and medical crisis. Provide services to clients in assessing need for and accessing necessary medical, behavioral health and supportive services. Provides comprehensive care management, care coordination, health promotion, transitional planning and follow-up, individual and family support, and referrals to relevant community and social support services. Skills Required • Uphold client-first mindset by appreciating the client’s uniqueness, personality, culture, and interests and considering client’s expertise. • Support and uphold the mission, vision, and values of Community Psychiatric Clinic. • Help client identify and establish individual goals. Help the client to work towards stated goals. • Connect client to other professionals and supports to establish a coordinated plan of care. • Experience assessing for risk factors, health status, self-management skills, and confidence level. • Must be able to navigate and manage multiple confidential health care information systems. Job Requirements • BA degree in a related field, RNs, ARNPs, LPNs, Social Workers, LMHCs, CDPs, or Peer Counselors with related experience. • Experience working with individuals with co-morbid chronic condition management as well as behavioral change methods; experience in medical, mental health, or social services. • Experience in convening and facilitating multi-disciplinary teams. • Reliable transportation is required. • Experience with electronic medical records is preferred.
