CPC is seeking a part-time Nurse Care Manager who will work alongside our multidisciplinary team which delivers high quality, evidence informed, and community-based behavioral healthcare services to our clients. The Nurse Care Manager is an essential member of a team of substance use disorder, behavioral health, primary care and other partnered providers who together providing access to a continuum of services for persons with opioid use disorders, utilizing a “hub and spoke” model and a harm-reduction philosophy of care. This position will provide assessment, education, stabilization, nursing care, care planning, support to clients and medical support to prescribing providers. Job Functions: Provide medical support to Medication Assisted Treatment prescribing providers. Conduct screening of clients interested in medication assisted treatment. Provide client education about medication assisted treatment. Assist with buprenorphine inductions. Obtain vital signs, urine drug screens and basic labs. Conduct physical health assessments. Document in the electronic health record and treatment registry. Provide care planning to clients. Participate in stabilization and maintenance phases of medication assisted treatment of clients. Provide ongoing coordination of follow-up care. Provide relapse prevention support to clients. Other duties as assigned at the discretion of management. Job Requirements: Active Washington State RN license. Current First Aid and CPR certification. Competency at basic nursing skills such as venipuncture, IM injections, vital sign monitoring. Basic computer literacy and familiarity with use of an electronic health record. Familiarity with the use of Medication Assisted Treatment of substance use disorders and with the harm reduction approach to substance use disorder treatment. Familiarity with community resources for behavioral health, substance use disorders, affordable primary care. Preferred Qualifications: Experience with psychiatric and substance use disorder client populations. Experience with case management or care coordination.
