CPC is seeking a Veterans Program Case Manager who will work alongside our multidisciplinary team which delivers high quality, evidence informed, and community-based behavioral healthcare services to our clients. Veterans Program Case Manager will provide outreach and other direct and indirect services for homeless Veterans and Veterans who are at risk of homelessness in the Supportive Services for Veteran Families (SSVF) program. CPC's SSVF Program: Community Psychiatric Clinic has been awarded a federal Veterans Administration grant to initiate an exciting new Supportive Services Program for Veterans Families (SSVF). This program will target single adult veterans and veteran families who are literally homeless or at immanent risk of becoming homeless. The SSVF program coordinates with State, County, and Federal veteran serving agencies and other service providers to provide case management, homelessness prevention and rapid re-housing. Qualification Requirements:

Job Requirements: B.A. in social services or a related field. Have or obtain a Washington State License or Agency Affiliated Counselor Registration. Two years direct clinical experience preferred. Two years human service experience, preferably with Veterans or community partnership or assessment/screening services preferred. Licensure Requirements: You must have, or obtain, a Washington state Department of Health counselor registration, certification, or license prior to beginning work.
