CPC is seeking a Community Outreach Worker who will work alongside our multidisciplinary team which delivers high quality, evidence informed, and community-based behavioral healthcare services to our clients. Full-time position, assessing CPC clients who are currently hospitalized or incarcerated at any King County hospital or correctional facility. Duties include coordinating with jails and hospitals to streamline follow-up mental health services, providing short term crisis intervention and counseling, and communicating with primary CPC clinicians regarding status of their clients in these facilities. Must be able to maintain good rapport with managed care referral contacts and individuals or organizations concerned with managed care; be able to deal with diverse clients experiencing a wide range of problems and be able to appropriately intervene on an individual, family or group level. Must have proven assessment skills to determine client functioning, and demonstrated ability to work effectively with the medical model and DSM diagnostics. Job Requirements A Bachelor's Degree in social work, counseling, or a related field. A minimum of two years direct clinical experience. Have or obtain a Washington state counselor registration, certification, or license. Have and maintain a valid Washington State Driver’s License, a have access to a vehicle for use during working hours.
