CPC is looking for a full-time FCS Housing Case Manager who will work alongside our multidisciplinary team which delivers high quality, evidence informed, and community-based behavioral healthcare services to our clients. The FCS Housing Case Manager is an essential team member of the CSS program, assisting those clients currently in service and those referred for Foundational Community Support to move out of homelessness and into permanent housing and to address any barriers related to housing permanence. The Foundational Community Support case manager works closely with prospective landlords, partner landlords, case managers, and the CSS Program Supervisors, as well as with participant households as needed. The FCS Housing case manager is a key player in identifying housing options for FCS program participants, working to develop connections to available housing resources and streamlining this function across the team. Job Functions: Performs searches for affordable rental housing options, for CSS clients and those referred to FCS and identifies appropriate resources for housing placement. Provides updates regarding housing openings, housing placements, and potential housing options, as needed to CSS team members; regularly attends and utilizes supervision, consultation, and team meetings to coordinate and share housing resources and makes appropriate connections to landlords for FCS participants. Provides outreach to landlords, community housing providers, rental groups and housing agencies to recruit and retain housing partners. Provides education to, and makes connections with, housing providers to partner with CPC with the mutual goal of housing FCS participants. Documents ongoing housing resources and outreach performed; creates, maintains, and regularly updates tracking system with detailed information to assist in matching program participants with housing options and available properties that are appropriate. Maintains complete and accurate files on landlords, housing options, housing placements, and other files as required. Maintains communication between landlords, program staff, the program supervisors, and other affiliates of participants and the larger community;

 

Qualification Requirements:

Job Requirements: Bachelor’s degree in social work, counseling or a related field. Two years’ human service experience. Experience working with housing or community support services. Have and maintain current Washington State DOH counselor registration or license. Have and maintain current first aid and CPR certification. Have valid Washington State Driver’s License and acceptable driving record. Preferred Qualifications: Two years’ Housing Case Management experience. Any job offer will be contingent upon the results of an updated background investigation.
